Home Health Services

To qualify for home health services under your physician's supervision, one or more of the following conditions must have occurred.

  • Homebound: If you get sick or get worse
  • Recent hospitalization
  • Recent new or exacerbated diagnosis
  • Change in medications in the last 60 days
  • New medication in the last 30 days
  • Change in primary caregiver providing care with a knowledge deficiency
  • Treatment of an illness or injury that must be performed by a nurse
  • Medication administration (other than oral)
  • Wound Care
  • Urinary catheter care
  • IV Therapy
  • Diabetic care
  • Recent marked decline in functional status (i.e. speech, ambulation, strength, endurance)
  • Recent falls, fracture, and stroke
  • Need for home maintenance program to maintain current level of function

Physical Therapy

Our Physical Therapists work to increase your strength and mobility. We provide education on techniques to promote increased activity tolerance and independence following your illness or injury.

Physical Therapy services can include such things as:

  • Therapeutic Exercises
  • Gait Training
  • Transfer Training; and
  • Developing a Home Exercise Program

Occupational Therapy

Our Occupational Therapists will assist you in regaining independence in your activities of daily living, which includes teaching you:

  • Self-Care Skills
  • Pacing and Breathing Techniques
  • Caregiver Training / Patient Care at Home

Our therapists will teach you new strategies and techniques to help you with your transition to wellness after serious illness or injury. Our therapists' goal is to help you restore your skills and to function at the highest practicable level.

Speech Therapy

Our Speech Therapists will work with you to help restore your valuable communication tools.

If you are having difficulty speaking, hearing or understanding others, our therapists can help. We work on all aspects of cognition including memory, sequencing and problem-solving. In addition, we help treat problems with swallowing and oral motor control.

Conditions which can be treated with this type of therapy may include:

  • Involuntary Stammering/Stuttering
  • Tourettes Syndrome
  • Cognition Deficiencies
  • Dyslexia
  • Swallowing Difficulties
  • Delayed Speech

Skilled Nursing

Registered Nurses and Licensed Practical Nurses (LPN) provide skilled nursing services. Our Registered Nurses evaluate your needs and help establish a plan of care under directed by your physician. Our LPNs provide follow-up care that will teach you and your family about ongoing care to promote independence.

Our skilled nursing staff will educate you regarding:

  • Disease Management
  • Medication Management
  • Dietary Management
  • Disease Prevention; and
  • Symptom Control

We also provide IV Therapy, Ostomy Care, Wound Care and Catheter Care.

Medical Social Services

If you are discharging from the hospital, a Social Worker can arrange for post-hospitalization care in your home.

Social Workers can be a source of emotional support, especially when you and your family are dealing with a chronic disease or a life-limiting illness. Our Social Workers assess your needs and can provide you with local resources for a variety of services that you may qualify for.

Private Duty Nursing

Desert View Home Health, Inc. is dedicated to providing skilled pediatric nursing services to medically fragile children. Skilled Nursing Care Services for Pediatric Private Duty Nursing include, but not limited to:

  • Ventilator
  • Tracheotomy
  • Gastrostomy/Nasograstic Tube
  • Central Line
  • Catheter
  • Ostomy
  • Total Parenteral Nutrition
  • Wound Care/Complex Dressing Changes
  • Medication Administration (IV’s, IM, SQ, GT/NGT, etc.)
  • Family Education
  • 24/7 Hourly Private Duty Care

Pediatric & Neonatal Nursing Care - RNs & LPNs

Our nurses are proud of their reputation as qualified , pediatric home care providers, and, at a minimum, have:

  • Previous pediatric experience or have completed pediatric care training
  • Current state licenses
  • Criminal background clearance
  • CPR/Emergency/First Aid Training & Certification

Our Certified Nursing Assistants are trained to deliver home health care services which are suitable for various clients and are there to provide you with the assistance that you require as you recuperate in the comfort of your own home. They assist you with of daily living such as, bathing, dressing, light housekeeping and meal preparation.
